Teacher Referral Form (TRF)

This brief form can be included in the online ATDP application in the following ways:

students can list their teacher on their application form, which will send an email request to their teacher (recommended)
students can use this form to send an email request to their teacher before they begin an online application
teachers can complete this form themselves, which will email a code to the student's family that they can use when they apply online
